Outpatient Services

After completing their inpatient stay, individuals have the option to return to our services as an outpatient and continue their rehabilitation in one of our dedicated neuro rehab gyms. During this next phase, patients work with our expert therapy team to set new goals and build on the progress they’ve already made. Treatment plans are fully bespoke and tailored to each person’s unique needs and aspirations. 

Patients can choose the frequency of their visits or engage in an intensive therapy package, depending on their individual circumstances. Throughout their time in the gym, progress is closely monitored by our therapists, with regular reviews to adapt and advance the treatment plan. Therapy continues until each individual reaches their maximum functional potential.

Residential and Supported Living Services

We operate multiple services based throughout the UK, specialising in brain and spinal injury, epilepsy, learning and/or physical disabilities, often involving high-dependency ventilation and respiratory care and mental health. 

At our services, we provide progressive, personalised care pathways that combine the latest rehabilitation technology, clinical expertise, personal support, and therapeutic services, in a range of different settings to meet the evolving needs of those we support. Neurologic Music Therapy®

Neurologic Music Therapy®

Music is a powerful tool in our rehabilitation and is delivered in partnership with Chroma. Through Neurologic Music Therapy®, we help people regain movement, communication, and thinking skills, using music to support medical and emotional recovery goals in a way that feels motivating and personal. For patients with disorders of consciousness, or where awareness is uncertain, we also use the MATADOC assessment. This helps us identify signs of responsiveness that might otherwise go unnoticed, guiding the team towards the most suitable treatment plan and helping families better understand what’s possible.

Hydrotherapy, taking place in a warm water pool, enables individuals to engage in exercise activities that they may not be able to do outside of the pool. Doing so can alleviate symptoms, promote flexibility in joints, improve muscle strength and endurance, and enhance functionality and independence.

Our Therapeutic Model of Care

Our model of care goes beyond the minimum therapy requirements to provide consistent, high-quality support that integrates proven practices with innovative, tech-enabled tools. We believe that rehabilitation happens 24 hours a day, 7 days a week with every interaction, not just with scheduled therapy sessions. With a focus on measurable outcomes and meaningful change, our model ensures that each step forward is intentional, supported, and sustained.

Our 4-stage model of care ensures personalised care every step of the way.

Step 1: Pre-admission

Setting expectations:

  • Enquiry is received and assessments and reports are completed within 72 hours

Step 2: Admission and Assessment

Rebuilding the rehabilitation framework (up to 2 weeks):

  • Initial assessment completed by our Multi-Disciplinary Team (MDT).
  • Goals and initial outcome measures are collected.
  • Progress meetings are scheduled for 3, 6 and 9 weeks depending on Length of Stay.

Step 3: Treatment

Delivering high impact rehabilitation (0-12 weeks):

  • Up to 4-5 hours of therapy per day, across the week.
  • Technology-based rehabilitation and hands-on therapy techniques.
  • Personalised and goal-focused care.
  • Weekly ward round and team meetings to assess progress and adjust treatment plans if required.
  • Regular progress reports.

Step 4: Discharge & Beyond

Measuring impact & ensuring continuity (12 weeks):

  • Discharge outcome measures collected.
  • Discharge planning to ensure a smooth transition home or to the next stage of care.
  • Referrals to community services are arranged when needed, to support care after discharge.
  • Support is available after discharge through Active Care Group, including outpatient and our Care in the Home services.
