Our therapeutic model of care.

Rehabilitation does not stop at the therapy room door. Our inpatient model builds recovery into every hour of the day, and into everything that comes after it. From pre-admission through to discharge, your Navigator coordinates the plan, aligns clinical and personal goals, and stays a constant point of contact for you and your family.

Pre-admission

Enquiry received, with assessment and reports completed within 72 hours. Your Clinical Navigator is assigned before you arrive.

Admission

A welcome, not a check-in. Your Concierge greets you and settles you in, and our multidisciplinary team completes the initial assessment.

Treatment

A bespoke therapy programme of typically 20 to 30 hours a week, set by your clinical presentation and funding pathway, combining technology-based rehabilitation with hands-on therapy.

Discharge

Outcome measures compared against your admission baseline. Discharge planning starts early in your stay, with the option of continuing as an outpatient considered.

Onward pathways

Outpatient access with us, care in the home through Active Assistance, or supported living through Active Care Group. Same record, same point of contact.

A single named point of contact whose focus is coordination, continuity and removing the barriers that slow recovery down. Because everything sits within one organisation, there are no repeated assessments and no handover gaps. No-one ever needs to tell their story twice.

Your Concierge
A welcome, not a check-in

Your Concierge

Your Concierge greets you on arrival, takes you to your room and settles you in. You then meet your Clinical Navigator, who talks you through your pathway and what it will involve.

For families

You are part of the team, not a visitor, and we will keep you close throughout.

Your Navigator keeps you and your family updated throughout, with regular progress reports and progress meetings at 2, 6 and 10 weeks, dependent on length of stay. Discharge planning begins early, not at the end, and your Navigator leads the conversation about where you go next, with you and your family.

Onward care pathways

Discharge from Active Neuro is a step in the journey, not the end of it.

Your Navigator plans what comes next long before you leave, so rehabilitation continues without a pause. Active Care Group provides outpatient therapy, specialist residential care, supported living and clinically led care in the home within one organisation, so moving on does not mean starting again.

Active Neuro outpatient and overnight services

Rehabilitation can continue on an outpatient basis, with regular therapy sessions while you live at home and the option to stay overnight if you live further away or need a more supported transition back home. Available at selected sites.

Residential and supported living services

CQC and Care Inspectorate registered specialist residential homes across England and Scotland with 24/7 support, and independent supported living flats with individual tenancies, building independence through everyday living skills.

Specialist care in the home

Going home is the goal for most clients, and Active Assistance is what makes it possible. Rated Outstanding by the Care Quality Commission, with packages from a few hours a week to full 24-hour support and consistent care teams who know you.

Fees and funding

Whoever is paying, the care is the same.

Whether a family, an insurer or a commissioner is funding the placement, the pathway and the quality of care are the same. Fees depend on clinical need and length of stay, so we quote in writing after assessment. Assessment and reports are completed within 72 hours of enquiry.

How quickly can someone be admitted?+

Assessment and reports are completed within 72 hours of enquiry. Admission then depends on bed availability and, where relevant, funder approval.

How long do people stay?+

It depends on the complexity of need and the goals set on admission. Progress meetings are scheduled at 2, 6 and 10 weeks, dependent on length of stay, and length of stay is reviewed at each one.

Can family visit?+

Yes, throughout the stay. Please call the centre to agree a time so it fits around the therapy timetable.

Can I come as an outpatient?+

Yes. Outpatient programmes, including outpatient with optional overnight stays, are available at selected sites.

What happens at the end of the stay?+

Your Navigator leads the conversation about where you go next, with you and your family, and arranges referrals to community services or onward care with Active Assistance where needed.
